Probably several things in getaddrinfo() library and friends.
I made just pure 2.99.54 package for you which has accumulated
some 350 lines of ChangeLog since 2.99.53.
You can now create a test-tool for those pesky MX things:
cd transports/smtp
make getmxrr-test
mv getmxrr-test /into/some/path/getmxrr-test
Then execute it with interesting destination domain.
The algorithm inside it is exactly the same as what smtp uses.
(But I do believe that I have solved the problem.)
