I think this happened when someone from a domain that used to be listed in
the smtp-policy.spam file tried to send mail to postmaster complaining
about it. I think the same thing happens with an RBL-blocked address, but
I can't be sure.

Postfix waits until RCPT TO: to error out, for RBL-listed addresses and
relay denial anyway.

Hmm, that reminds me - one nice feature of Postfix is the ability to
notify the postmaster when relaying is denied. The SMTP conversation is
sent to the local mail system admin. (Well, ok... it's nice at first, but
to continue using it I have to keep filtering persistently retrying MX IP
addresses in my .procmailrc.)
