smtp-policy configuration help?

I am wonder what exactly do I have to do to enable the smtp policy
stuff. I ran the policy-builder.sh, enabled the "PARAM policydb"
line in smtpserver.conf (and changed the database type to gdbm),
and restarted the smtpserver. But I still get junk mail from sites
that are blacklisted.
